You acknowledge the following:
• Carson Meyer, C & the Moon, Inc. and their affiliates (collectively the “Company”) will provide [nutritional counseling services / doula counseling services] (the “Services”) to you. Company’s agreement to render the Services does not create a patient-provider relationship
• The Services do not include diagnoses of medical or mental conditions, nor do the Services substitute for medical care, mental health services, or the practice of medicine by a licensed physician. Rather, the Services are intended to help develop an appropriate health-supportive program and to monitor progress in achieving goals. If you are in need of or are otherwise seeking personal medical advice, you should consult with the appropriately qualified health care provider. If you think you may have a medical emergency, please seek assistance immediately.
• Company makes no guarantees, representations, or warranties of any kind or nature, express or implied with respect to the Services. In no event shall Company be liable to Client for any indirect, consequential or special damages. If you choose to rely on any information provided by the Company, you do so solely at your own risk.
• You understand and agree that you are receiving the Services voluntarily and that you may stop such Services at any time.
